The short answer
Bring a photo ID, your insurance card if you have one, a list of current medications, and any recent imaging reports (X-ray, MRI, CT) related to your concern. If you completed intake through the secure portal ahead of time, that is everything.

- Photo ID — driver’s license or similar.
- Insurance card — if you plan to use insurance. Self-pay visitors can skip this; see the payment guide.
- Medication list — names and doses, including supplements.
- Imaging reports — the written report matters more than the disc; bring both if you have them.
- Your questions — written down, so the visit answers what you actually want to know.
What not to bring
You do not need to bring completed paper health forms downloaded from the internet. Intake is completed through the office’s secure process so your health information is handled properly.
